PLEASE STATE A REASON on any on-line booked appointments.
Please note that any on-line appointment booked will be contacted at anytime during the day of appointment - ie if you book at 09.00 you may be called anytime up to 18.30 on that day - If You require a certain time please detail this along with reason for appointment, thank you.
If you wish to see a doctor or other member of staff for treatment or advice, please telephone, book online (via the link at the top of this page) or call in to make an appointment. Please ask if you wish to see a particular person. We will do our best to keep appointment, but emergencies do occur.
If you need to see a doctor urgently, please explain this either when you telephone or call at the surgery. Urgent cases will be seen on the same day, but please do not abuse the system. We do offer a number of ‘book in advance’ appointments, a limited number of 1 week in advance, and a limited number of book 24 hours in advance.
Medication Reviews - Please book your medication review with the Practice Pharmacist and not the GP - thank you.

Telephone Advice/Consultation
If you feel you need to ask the doctor’s advice by telephone please ask the receptionist for a “Telephone Consultation”.
You can ALWAYS speak to the doctor in an EMERGENCY.
Extended Hours
Extended hours are held at the Croft Medical Centre, 1 Pomeroy Way, Chelmsley Wood, B37 7WB 18.30 - 20.00 Monday to Friday, and 09:00 – 17:00 Saturday, by prior appointment only.

Home Visits
If possible please try to telephone reception before 10:30 if you require a home visit.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.
You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

Cancel an Appointment
It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.
You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays). You can also text to cancel your appointment.
Text Reminder Service
We have a texting service which allows you to receive confirmation and reminders about your appointments.
To have this service you will need to register by completing a consent form.
Please remember to update your contact details with us when you change address, telephone numbers and email address.

GP Registrar
The practice is a teaching practice and from time to time for periods of up to a year, a trainee G.P may join the practice. This doctor will be fully qualified and is gaining experience in General practice before pursuing a career as a G.P; they will be involved in all aspects of work at the surgery and may offer special expertise.
Medical Students
At times Medical Students may be sitting in with a doctor. If for any reason you do not wish to see the doctor in the presence of a Medical Student, please inform the reception Staff.
